Choosing the right home or facility for your loved one is one of the most difficult decisions you'll ever make.
This is not an easy time for you and your family. Let us help you through these difficult times. As Registered
Nurses/Licensed Social Workers we are available to help you and your loved ones. We will meet with you and your loved ones to complete an
assessment that only a Registered Nurse/Licensed Social Worker can provide.

How Does the Process Work?
The placement process begins with a phone consultation to help determine you or your loved ones needs.
We will meet with you and your loved ones to discuss choices and to complete an RN/MSW assessment. Part of this
assessment is to educate you and your loved ones on senior living resources that are available.
After this assessment and the care level needed has been determined, we will drive you to various care settings
that meet your criteria. We will also assist with the transition into the new home.
During the Transition
After the move in, A+ Nursing will do a no cost follow-up visit within the first two weeks to assist with any care needs.
We will follow up with you to let you know how your loved ones are doing. We will also continue to be available to assist
with any care needs.
